To recreate error, kindly follow the following steps;
Login to site: sms.LilongwePrivateSchool.com (As this is a forum, I have already sent the User ID and PWD of the student via email)
Click on Homework  Submit Homework
Attach a file with a description and click Submit.

At this point the above error is regenerated.
